COVID Vaccine Consent Form

We now offer COVID-19 RT-PCR Testing offered in partnership with ProPhase Labs. This time, we only provide the RT-PCR type testing.

The COVID-19 test is a Real-Time Polymerase Chain Reaction (RT-PCR) test for the qualitative detection of RNA from specimens (such as nasal swabs) collected from individuals suspected of/screened for COVID-19. This is highly accurate testing that targets multiple gene mutations of COVID-19. This testing is the test type required for travel to many countries abroad.

Which COVID-19 tests are required for international travel? It depends on where you’re going. To limit the spread of new coronavirus variants, many countries are requiring incoming travelers to show a recent negative test. You 1st Pharmacy will be able to provide you test result documentation.

Many countries will accept results from either a test that detects the genetic material of the virus — considered the most sensitive type of test — or a rapid test that looks for viral proteins called antigens. The tests must have been taken no more than three days before departing for travel abroad.

There are two types of tests for COVID-19. Viral tests tell you if you have a current infection, and antibody tests tell you if you’ve been previously infected. Our test is the viral form of detection.

If you’re experiencing symptoms or think you’ve been exposed to COVID-19, call us in advance to schedule your PCR test at 980-320-0160. Walk-ins are welcome for pre-travel testing.

****Proprietary PCR test indicates both positive and negative as well as INVALID CONTROL which avoids false negatives due to invalid collection****

Even though people may be vaccinated, they still may be required to continue testing. As part of the health care long-term normalization for the United States, Covid-19 testing will continue to be a focus. Many organizations, in their efforts to return to active engagements, will continue to require Covid-19 testing. This includes schools, camps, places of worship, sports and entertainment events, travel, and other places of gathering.

Testing is at NO COST to a customer in many cases.

The lab will bill either insurance, Medicare, Medicaid, or if lack of insurance, the government under HRSA.
